What does "+X% to All Damage" means in a skill?

Let’s take for example Markovian’s Advantage skill from Soldier: Markovian's Advantage (Skill) - Official Grim Dawn Wiki

I’m assuming it means that it grants my character a passive buff of +15% to All Damage, but I began to question myself if it actually meant: “Once the skill procs you do +15% to All Damage for this hit”

So which is it?

It only affects the skill itself, so whenever it procs.
