After a long battle to get this achievement, I finally got 100% of the requirements, but still can’t get past 99% happiness. In the happiness details window, shoes and clothes are reduced because of infants who don’t wear them but in the town hall, they are not counted against me.
The one “worms” infection is on a 1 year old child that cannot wear shoes or clothes, and the house has only 5 waste buildup.

This is a difficult achievement and you simply haven’t done it right. Not only your shoes and clothes, your cleanliness and entertainment are not 100% so you won’t get this achievement.
The key here is: Villagers only have children (including new immigrants) when there is room in the house.
There is also another way shown in the image I provided.
Your population shows 601/668 with the warning icon that you are over the set population limit which tells me you lowered the population limit to prevent new births. I’m not sure that is what was intended when they set up the achievement requirements though because they did intend for the 500 limit to be the default for game stability among all systems and requiring a player to push past this for an achievement seems unfair to some with less performant systems.
Yes, that’s the second way and the way I used (simply because I wanted to save a little more time to move on to another goal - conquering the Raging Raiders mod)
Obviously, the population limit setting feature was created for lower-spec computers to be able to run the game without too much lag. However, using this customization will not require waiting for the villagers to fill the houses so that new children will not be born. Otherwise, you simply need to wait a little longer for the villagers to fill the houses.
The real challenge of this achievement is cleanliness. Getting 100% is not easy but it is very interesting.
BTW: the max population limit feature has multiple levels, 200, 500, 1000…
As far as I knew, the lowest you could set the max limit to was 500 but okay. I guess I never tried to slide it down from 500 before. You’d think if they were going to set specific figures for this, they’d use a radio button instead.
I noticed that when the houses start to all become full, your town hall will tell you that villagers are feeling over crowded. I’m concerned this might have a negative impact on happiness.
