I think they’re chaos totem drops. I think it was mentioned in one of the Grim Misadventures, but I could be misremembering. Or confirmation bias as I’ve gotten them from totems.

btw, anyone considered voidheart/entropic coil or 2x voidheart with the build?

I theorycrafted one with Myth Combustion band and Voidheart. Actually excited about this one, I’ll start levelling it on the weekend. 
Now I gotta check my stash to see how many I have. 
It was bugging me that I had no evidence to back up my claim that Voidspire is a totem drop, so I did some sleuthing.
In the 1.1.6.0 patch notes, Zantai stated, ‘Ten new Legendary quality weapons have been added to the game. These weapons can rarely drop from any source, but are more likely to appear from Monster Totems,’ and ‘Ilgorr the Eternal, from the Steps of Torment, and Lagoth’Ak Harbinger of Blood, from the Bastion of Chaos, have been rummaging through their possessions and discovered 4 new Monster Infrequent medals for you to collect.’
Grimtools corroborates that Voidspire and Ilgorr’s and Lagoth’Ak’s medals were added in 1.1.6.0. There is no mention of what type of totem drops Voidspire, but the shared theme of the weapon and totem leads me to hypothesize that Voidspire drops from Voidtouched (chaos) totems.

I think they drop from Cthonic totems more frequently, but they are also in global drop pool. Just quite rare outside of totems.
