Neither W key, nor hotbar button, nor inventory button work.
Bug is hard to pin, but it appears, and seems to disappear after using certain skills (latest time it was Vire’s Might).
I have main game + AoM, FG and FoA. Save file and log file are attached. What other information I can provide?

I’ve experienced this problem before too, but now I caught it on video:
Bug happens at 0:49 in the video.
This has been happening at least since the new DLC dropped, and particularly with Werewolf form. From what I’m experiencing it happens rarely every hour or so, but after I notice that I can’t swap weapons, I continue playing normally, and after a couple of minutes the bug is gone and I can swap weapons again.
In the video I just triggered a Totem, fought a Nemesis, and got Nullified once. And after all of that I can’t weapon swap anymore. I’m unsure if any of these 3 events are part of the cause or just coincidence.
Sorry for the double-post, but I’ve got new information.
I had to test this a little better, and indeed the bug gets fixed in Werewolf when using Rip and Tear (which is basically Vire’s Might).
1 minute video of weapon-swap working; weapon-swap bugged; weapon-swap working:
So Rip and Tear absolutely fixed the bug, instantly. I did cast Rip and Tear multiple times during the fight, so maybe it’s the cause too?
Edit: I noticed that the game (intentionally?) doesn’t let you weapon-swap mid-way through a charge skill like Rip and Tear. So probably the game still thinks the character is still doing it’s Rip and Tear run, even though it already ended. Checking this video frame by frame, when I cast Rip and Tear for the last time on the final enemy, I was at melee range so my character didn’t move at all. But there was no stun, freeze, or any CC happening at that time that could cause this. Very strange.

Hmm, that’s something that can be tested. I specced out of Vire’s Might for Blitz, which doesn’t seem to be affected by the bug. But I could respec and see if using Vire’s Might at point blank range causes the bug.
UPD: tried it, doesn’t seem to trigger the bug on its own. There must be multiple conditions then…
I confirm I experience the same as a shieldbreaker on 1.2.1.2, so old bug. Certainly related to Vire’s Might.
