what does “Chance of Reduced target’s Resistances” exactly mean - does it mean that every time I hit an enemy I get this chance? Same for spells, minions, retaliation?

Yes, exactly. Not sure about minions and retaliation, I’m guessing not but could be wrong.

When the effect is on a weapon and you dual wield - does it only go off, if you hit with that weapon and not with the other one?
