Unsure about the number of different visuals; at least two.

It’s Badge of Mastery. The 3d model for it is that same as the common star medal (I think that Black Star of Deceit and the PRM epic medal which name I forgot also use the same model).
A lot of legendary and epic medals use the common 3d models on the PC model. There are some that have a unique model, like Undying Oath, but that’s very few.
