if u having massive death it maybe be a plague due to mass rats and waste
Diseases @ 135 is a bit high for your population. I would say this is due to sanitation and/or lack of medicine (herbs, potions).
Check to see how much waste is in your homes. If you see a lot of them with 20+ waste, you need to add more Compost Yards.
Hey, my cleanliness is at about 94% because of missing barrels i guess. Unfortunately dying began after building new compost yards… So I have to spend decades to regrow 
1 compost yard per 100 villagers
Keep to that formula and you will be fine in the future.
