Finally solved my problem. The N’erf final boss follows the same fight mechanics like the insect queen underneeth the fields of Homestead. If you burst out damage and kill him “too fast” his second phase transformation DOES NOT trigger, leaving you with no boss to fight, no portal to leave and a bunch of locked loot chests. I started running around for a while, leaving my pets to do the damage, a little at a time, re-summoning them as they fell, until his first phase was over, and after he transformed then I joined the fight dolling out my burst damage and thus finally killing him for good.
alright, i’m giving up, i’ve been everywhere and can’t find the last one, i’ve spent hours uncovered every square on the map, 5/6
