Are you using a disc version or the Steam version? This following response is based off the ‘Retro’ disc version, but can still apply to the Steam version in some ways.
With you skills, I’d go to something looking more like this.

I took the 1 point out from crushing blow. The reason for that is because except for one point in the base skill, the Battle Rage line is pretty useless. The reason you put one point in Battle Rage is for the +%OA
I also took the 1 point out of Doom Horn. This is because Undeads and various other enemies have resistance to that life reduction so unless you were a Spellbreaker able to bring that down, it’d be not working against a decent portion of enemies you face.
Finally I took the 1 point away from Sleep. The reason for this is because when you hit an enemy, Sleep dissipates and given you’re constantly hitting your enemies, that makes it kind of pointless.
What I did with those three points was put a point into Cross Cut and Tumult in addition to putting another point into Hamstring.
In regards to your Trance, I think it’s a fair one to take for the ADctH, but I can agree Trance of Convalescence can be really great too.
When it comes to Health, you don’t really need to invest much into it. You’ll get more from items, but if you really feel you need it, you can invest the bonus stat points from the side quests (Chiron, Beating Aktaios, and The Bandari) Also remember you’ll get health from the Poisoned Springs again and from Lampido’s Potion each difficulty.
What are the prefixes on your gear? Your Sabertooth is a good find since those are pretty great swords with the right prefixes and suffixes. When it comes to it, Veteran’s is a great prefix on your weapons. You might also consider putting the Folg in your right hand on the toon in place of the axe depending on the Folg’s stats.
A good non-gear dependent caster can actually be found with Dream. It’s also the only viable DoT character that isn’t heavily gear-dependent. It’s based on the Electrical Burn Damage, or EBD, from Dream. Basically how you play the toon is to Cast your EBD skills and then put your enemies to sleep. (An already running DoT does not break Sleep.) Preffered endgame gear with Soulflay Fork (yes, a caster with a spear ), a shield based on your situation (I personally plan to go for Santa’s Peppermint as it only has a level requirement.) Legs would be Demonskin Walkers, followed with other stuff. (I believe Wings of Carnus set was recommended for your head and arms.) Violos is the creator of the build so compliments go to him. It works best as a Haruspex, but can be done with any of the masteries.
When it comes to more heroes, you could always try the MassBosses mod.

To use it you put it in your custommaps folder (create one if you don’t have one) in the Immortal Throne folder. You may need to extract the files. Then once on the main page you go to custom quests, select the MassBoss mod and click Start. That will go to a load screen and bring you to your main quest characters, at which point you start the toon and the mod runs. What it does is raise the occurrence of heroes and how many of them appear the same time. (For example, you can fight multiple Mulgor Flamespears at the same mob.) It also increases the spawn of champion monsters and normal monsters in addition to Boss Monsters. (Imagine fighting 24 Typhons at once. ) Note that a hero can only spawn once per session, so extended sessions will see heroes at the beginning and then see them drop off later on.
As for new heroes/bosses, there are a few new bosses. As Asylum said, only one isn’t hidden (as in you will find him while going to places you need to go in order to complete the main quest) but you’ve already been to where the others are I’m sure. (None of them are parts of new quests as there are no new quests.)
