Are you sure you’re not hitting the Weapon Swap keybind also?
Yes, unfortunately, the first and second images show the result of switching weapons, so in both cases, the character isn’t holding anything.
We cannot reproduce this. Wereforms do not have any effect on equipped items.
There has to be another factor.
Could you upload that build to Grim Tools / Calc? To see what items you have?

I forgot to mention this, but this phenomenon occurred when I died while in werewolf form and then respawned.
Are you using the gloves that can not be dropped in game?

That’s because I wasn’t out on an adventure; I was just testing which builds would create the strongest synergy.
I also encountered similar bug. While in wereform I cleared devotion shrine and unlocked rend celestial power from huntress. I tried assigning it to different skills to check out how its %chance to trigger changes. I decided to stick with judgment. I teleported to a different area and started fighting. After killing few enemies I noticed I hit like wet noodle. I chcecked equipement and noticed I don’t have anything in weapon slots. I thought that I must have hit swap weapons hotkey, so I clicked the swap weapon button in the equipment screen. The weapons didn’t come back. Out of frustration I clicked the button again multiple times and the weapons appeared back. I was dual wilding bonslicers btw.
