What next? 
Im not sure what to do or where to go or how the game progresses now, the best way to get good gear etc, what to farm if anything, i need some advice ^^
What level are you?
Your next option is to try the elite difficulty. If that is too much, then it is to go through Bastion of Chaos in act 4 to get some more leveling and possible items
47 - i just started on elite and having no real problems with regular mobs so far.
Should be fine then, but the Bastion of chaos (BoC) is there for some leveling up to 55 I think
Game starts at Ult difficulty. Go on.
