Some of the Kymon’s Chosen quests tell you to report to Father Kymon when the NPC you need to speak to is Brother Miranok:

This is not a bug, but a limitation of the game’s quest system, which was never designed for turn in NPCs to change like that.

The Forgotten Gods expansion introduces a scenario where the original NPC leaves, so a replacement takes their place.
