Can you explain to me how the modification given by the item works?
does it apply after the first part or does it change the first part by decreasing the targets from 5 to 3?
From my understanding, it makes you hit 8 target at once in 270 degree arc. I believe it doing something like this image below:
Ex: you’re surrounded by 10 mob and hit the NW red target. All 7 of the red target got hit by you, but the 3 orange target can’t get targeted because it’s out of the arc range despite you can attack 8 target at once.

Arivus is Correct. There are some skills that do not start with an arc (single target), but if an item or skill mod adds arc to the skill, it becomes an AoE, as in this case.
Brute Force by itself is only a single target. But when you put at least one point in Feral Hunger, it becomes an AoE with a 180* cone. The item adds 90* to that. the target is dead center, and the AoE extends 135* to either side of the main target/direction your facing.
