Starting Ashes of Malmouth

I have just downloaded Ashes of Malmouth (AM) and Forgotten Gods. I want to start playing AM but I can’t get the required item from Creed to start the DLC. I know Creed has to provide an intro and offer the item, but in my game I don’t get that option. Does the Forgotten Gods DLC assume you’ve done AM? How do I start AM?

To start AoM complete the main campaign and talk to creed at fort ikon

I’ve done the campaign once and am almost finished on veteran. Wont finishing throw me back to the start on elite level and I’ll have to do the campaign again?

you can keep playing on normal after unlocking elite but yeah to access AoM on a specific difficulty you have to finish the campaign on that difficulty
