I have, sadly, no proof or screen of this, and only vague ideas on how to recreate it, but here is what happened…
I was fighting the Guardian of Bysmiel in its cave with my Werewolf Reaver, I used 2 weapons and no alternate weapon set. Just as the Hellhound went down, the Beast Nemesis counter was hit and the Underking spawned. I noticed I was barely doing any damage to him, and so did a tactical retreat out of the cave. That is when I noticed my weapons were gone, the slots empty. Clicking the weapon swap icon, I got nothing happening. That persisted even after I equipped two weapons from my loot - clicking it did not switch to an empty slot.
I went back in and eventually killed the Underking, and then went to pick up the runestone of Bysmiel and the loot the Guardian dropped, and then suddenly, my weapons were back. The new weapons I picked up were put in the secondary weapon slots. Very weird experience. Not sure if it is related to the quest or the Underking…
Weird stuff like that has been mentioned before, but I do not believe at present anyone knows how it happens.
