Don’t know about the rest, could be grim dawn with 1300+ hours…titan quest with 700+, skyrim with 600-700. And no idea about d2 hours, but i’d say 1000 or so. Oh yeah and total war games for at least 3000+ hours, the most played being warhammer 2 with 750 hours. It’s very hard to list top 5 after all these years of gaming!
Um, not entirely sure on this for 5. In order of hours played:

TQIT/AE - 10,000+ over 10 years so far.

GD - between Steam and GOG versions I currently have just over 2,600, but that doesn’t count the hours spent in Steam early access. When I switched over to GOG I lost the Steam record for that, but probably a few hundred more to add to that total.
3-5. Can’t really say for these but - Zeus: Master of Olympus, Emperor: Rise of the Middle Kingdom, Children of the Nile, Port Royale 1 and 2, Anno 1503 and 1701, Caesar VI. Of those I would guess mostly Emperor and Children of the Nile.
